Early Intervention for Children (birth — age 3)/Central Referral Team (CRT) For children residing in Isanti County, birth—age 3, contact Cynthia @ 763-689-4071 to schedule an appointment. For children ages 4 and over, please contact the Early Childhood Center in your school district to make an appointment. The Central Referral Team (CRT) is the local Early Childhood Intervention team. It is designed to help families find health, educational and social services that promote the development of their eligible infants and toddlers. All children are eligible for an Early Intervention Assessment. Referrals to the program can come from parents, teachers or physicians.
Child & Teen Clinics
Child & Teen health check-ups include: * Complete physical exam * Immunizations/shots * Hearing check * Vision check * Lab tests * Checks on growth and development * Information about good physical health, mental health (thoughts, feelings and relationships) * Referral to a dentist * Fluoride varnish Child & Teen Check-ups are for Children and Teens, (birth to 21 years of age) who are enrolled in Medical Assistance and/or Minnesota Care insurance. Follow Along Program
Babies and young children grow and learn at their own pace. That said, some children need special help to grow up healthy and learn basic skills such as sitting, walking or talking. Let the follow along program help you know if your child (birth -3) is playing, talking, growing, moving, and acting like other children the same age.
Home Visits
New mom/baby home visits are offered to all Isanti County residents who recently had a baby, regardless of insurance. These visits are especially recommended for first time mothers, and usually take place within the first few days of discharge from the hospital. A new mom/baby home visit entails a one-on-one visit with a Public Health Nurse. During a visit the Public Health Nurse will take a health history on the baby including checking the baby’s weight, listen to their heart and lungs, and ask questions about the babies eating and sleeping habits. She will also take a health history on the mom and answer any questions or concerns she may have about herself and new little one. The visits usually last about 1 hour and a copy of the visit notes will be sent to both the mother’s and baby’s doctor, so they will know how each is doing before they come in for their first check-up.
Immunizations Isanti County Public Health provides a Children’s Walk-In Immunization clinic. To be eligible for the clinic, you must meet state of Minnesota requirements which state that you are not eligible to participate in a walk-in clinic if you have insurance that covers the cost of the vaccine with no out-of-pocket expense to you. If you have questions regarding your eligibility, call Cathy at 763-689-4071. A donation of $10.00 per immunization is suggested at the time of the clinic if you are able do to so. Please bring a copy your child’s immunization history record, or a letter from the school nurse detailing which immunizations your child needs for school. Outlook Clinic Outlook Clinic is a non-profit clinic, that provides low-cost, confidential health care services in the areas of family planning, birth control, sexually-transmitted disease testing and treatment and pregnancy testing. Outlook Clinic offers its services to women and men of any age and income level, serving East Central Minnesota. Clinic fees are on a sliding scale based on income. Contraceptive supplies and medications are available at a low cost. Personal Care Assessments (PCA)
The Personal Care Assessment (PCA) program provides services to persons who need help with day-to-day activities to allow them to be more independent in their own home. A Personal Care Attendant is an individual who is trained to help people with their basic daily routines. A Personal Care Attendant may be able to help a person with a physical, emotional or mental disability, or chronic illness or injury.
Pregnancy Testing Pregnancy tests are given free of charge on a walk-in basis from 8:00am - 12 noon and 1:00pm - 4:00pm, to clients of all ages.
