Correctional Health       

Isanti County Public Health provides nursing services to the  Isanti County Jail.  

 


Long Term Care Consultations

Long Term Care Consultations are provided by county public health nurses or social workers.  These visits occur in your home or long term care facility to help you identify services you might need or what and help plan for those services.  The visit is free of charge. 

A Long Term Care Consultant can help you:

                 *  Stay in your own home

                 *  Choose services you prefer

                 *  Move from a long term care facility

                 *  Understand your choices in services

                 *  Develop a plan to meet your needs

                 *  Access programs that help pay for services

Personal Care Assessments (PCA)

The Personal Care Assessment (PCA) program provides services to persons who need help with day-to-day activities to allow them to be more independent in their own home.  A Personal Care Attendant is an individual who is trained to help people with their basic daily routines.   A Personal Care Attendant may be able to help a person with a physical, emotional or mental disability, or chronic illness or injury.  

Isanti County Public Health provides Personal Care Attendant assessments for qualified individuals in Isanti  County.  To find out if you qualify or for more information on the Personal Care Assistance Program, visit the Minnesota Department of Human Services website at: www.dhs.state.mn.us

 


Senior Resources
